Job summary

Newport Mental Health seeks a Case Manager in Rhode Island to engage with adults 18+ and coordinate care to improve recovery outcomes. You will manage a caseload and work with a multidisciplinary team to address housing, transportation, and social support needs.

The role requires an associate’s degree (bachelor’s preferred) and 2+ years in human services, with strong communication and organizational abilities.

Qualifications

  • Associate’s degree required; bachelor’s degree preferred in Human Services or related field.
  • 2+ years of experience in the Human Services field preferred.
  • Valid Driver’s License with reliable transportation to travel to facilities.
  • Excellent verbal and written communication skills with diverse audiences.
  • Strong organizational skills and ability to meet deadlines.
  • Self-motivated, adaptable, and collaborative in multidisciplinary teams.
  • Problem-solving skills with the ability to apply critical thinking in client care.

Responsibilities

  • Engage with clients to reduce barriers and support positive outcomes.
  • Manage a caseload of adults 18+ with severe mental illness.
  • Participate in daily team meetings to review client visits and coordinate care.
  • Assist clients with recovery goals and obstacle planning.
  • Support with medication understanding and transportation, Social Security benefits.
  • Communicate with client families and housing authorities as needed.
  • Participate in supervision for therapeutic and problem-solving techniques.

WHAT YOU WILL BE DOING (ESSENTIAL DUTIES) …

We are seeking an experienced Case Manager to join our team. The Case Manager is responsible for actively engaging and collaborating with clients to reduce barriers and support positive outcomes in critical areas of life. The Case Manager will manage a caseload of adults ages 18 and older who require intense treatment. Clients commonly have severe and persistent mental illness.

Main Duties and Responsibilities:
  • Participate in daily team meetings to review client visits, concerns, and coordinate care.
  • Attend meetings with clients to assist with developing recovery goals with plans to overcome potential obstacles.
  • Provide support with understanding medications and find solutions to challenges such as transportation to appointments, grocery shopping or communication with Social Security for benefits.
  • Communicate with client families and support members as part of the client’s recovery plan.
  • Ensure client housing needs are stable, including facilitating applications with Housing Authorities.
  • Participate in individual or group supervision for therapeutic and problem-solving techniques.
Requirements

  • 2+ years of experience in the Human Services field preferred.
