We are seeking an experienced Case Manager to join our team. The Case Manager is responsible for actively engaging and collaborating with clients to reduce barriers and support positive outcomes in critical areas of life. The Case Manager will manage a caseload of adults ages 18 and older who require intense treatment. Clients commonly have severe and persistent mental illness.
