Job summary

Provive Wellness of Brentwood is seeking a Case Manager to support clients in our Partial Hospitalization Program (PHP) and Intensive Outpatient Program (IOP). This role is central to coordinating each client's care journey, connecting clinical treatment with the practical resources and support needed for lasting recovery.

Responsibilities include caseload management, intake, discharge planning, and level-of-care transitions, in collaboration with clinicians.

Qualifications

• Bachelor's or Master's degree in Social Work, Counseling, Psychology, or related field preferred.
• Experience in case management within a behavioral health, PHP/IOP, or substance use treatment setting preferred.
• Strong organizational, communication, and interpersonal skills.
• Knowledge of community resources and levels of care within behavioral health treatment.
• Comfortable working with clients experiencing mental health and/or co-occurring substance use disorders.
