Coleman Health Services, a nationally recognized not-for-profit mental health and substance use services provider is seeking a full time Case Manager to provide case management services to individuals with severe and persistent mental health issues who may also have a substance abuse disorder in Logan and Champaign counties. The Case Manager will assist with accessing community‑based resources, housing, employment and linking the individual with ongoing mental health and/or substance abuse services. The Case Manager will work with Probation, Parole, Courts, Social Security, DJFS and other providers. Must have BA in related field, one-year related experience and knowledge of community resources.
