Job summary

Marie’s House of Hope, Inc. is seeking a Case Manager to develop and coordinate programs with foster parents and adoptive families, and youths in independent living programs. The role involves training, fostering/adopting families, and teaching independent living skills.

Responsibilities include direct client training, placement interviews, coordination of services, weekly meetings with youth, and progress reporting to referring agencies. A flexible schedule and crisis coverage are expected.

Qualifications

• Education: Bachelor or Masters degree required.
• Experience: 2 years combined education/experience can substitute minimum.
• Field: Social work as minimum expertise.
• Preferred: 2–3 years in social work.
• Preferred field: social work in a business or industrial environment.

Job Summary

These positions participate in developing and coordinating Marie’s House of Hope, Inc. program, working primarily with foster parents and adoptive families and Children in the independent living program. General Accountabilities

• Provide direct training services to clients
• Interview prospective youths and determine appropriate placement.
• Supervised youth in placement and coordination of community resources.
• Meet weekly with youth
• Orientation of youth
• Development of treatment goals
• Coordinate educational, medical, recreational, and mental health services within the community as deemed appropriate.
• Write monthly progress reports based on the treatment goals to the referring agency.
• Complete special incident reports to Executive Director.

• Complete fifty (50) hours of training per year related to professional development.
• Carry a caseload of 4-12 Residents.
