Job summary

Community Rehabilitation Center (CRC) is seeking a compassionate and motivated Case Manager to join our Safe Horizon Program. The Case Manager will provide intensive case management, care coordination, advocacy, and community resource linkage for individuals experiencing homelessness or at risk of homelessness, including individuals with substance use and/or co-occurring disorders.

This is a grant-funded position; employment is contingent upon continued grant funding.

Qualifications

  • Bachelor's degree in behavioral health, social work, human services, or related field preferred.
  • At least one year of experience providing services to individuals experiencing or at risk of homelessness.
  • Case management and/or care coordination experience preferred.
  • Experience working with individuals with substance use and/or co-occurring mental health disorders preferred.
  • Knowledge of community resources, housing programs, and public benefit programs.
  • Harm reduction and motivational interviewing experience preferred.
  • HIV 500/501 training, screening, counseling, and testing experience preferred.
  • Strong verbal, written, organizational and documentation skills.
  • Ability to work independently and collaboratively as part of a multidisciplinary team.
  • Bilingual English/Spanish preferred but not required.

Responsibilities

  • Provide comprehensive case management and care coordination for assigned program participants.
  • Assist participants with accessing benefits and resources, including SNAP, SSI, SSDI, housing assistance, shelters, and other community programs.
  • Connect participants with behavioral health, substance use, medical, housing, and supportive services.
  • Work closely with CRC counselors, program staff, community agencies, and partnering organizations to coordinate care.
  • Monitor participant progress and successful linkage to community resources.
  • Maintain accurate and timely case notes and program documentation.
  • Collect required program data utilizing SAMHSA-approved GPRA tools.
  • Participate in required SOAR training and other grant-related trainings and meetings.
  • Conduct follow-up and discharge interviews with program participants.
  • Participate in community outreach activities, health fairs, forums, and other community events.
  • Maintain strict participant confidentiality and comply with HIPAA, CRC policies, and grant requirements.
  • Assist with meeting required Safe Horizon program goals, deliverables, documentation, and outcomes.

Community Rehabilitation Center (CRC) is seeking a compassionate and motivated Case Manager to join our Safe Horizon Program.
