Job Type: Contract to Hire

Schedule: Full-Time; M-F 9a-4p

Job Summary: Seeking a dedicated and compassionate Case Manager for Healthcare Navigation to join a dynamic team. This role is crucial in ensuring that Veterans and their families have efficient and effective access to healthcare services. The ideal candidate will work closely with Veterans, their families, and community providers to coordinate and deliver comprehensive healthcare support.

Key Responsibilities:

  • Connect Veterans to VA healthcare benefits or community healthcare services when they are not eligible for VA care.
  • Coordinate referrals to VA, community health clinics, and other programs to ensure Veterans have access to healthcare.
  • Develop relationships with community leaders, VA staff, and other referral networks.
  • Complete COVID-19 screenings in the HMIS system.
  • Manage a caseload of Veterans in need of healthcare navigation.
  • Provide care coordination and health education to Veterans.
  • Act as a liaison between the organization and the VA or community medical clinics.
  • Collaborate with the Veteran’s multidisciplinary team, including medical, nursing, administrative specialists, and case management personnel.
  • Assist Veterans in communicating their preferences in care and personal health-related goals.
  • Identify systemic barriers within the organization and work collaboratively to find viable solutions.
  • Act as a health coach by proactively supporting Veterans to optimize treatment interventions and outcomes.
  • Monitor Veterans' progress, maintain comprehensive documentation, and provide information to treatment team members when appropriate.

Case Management Responsibilities:

  • Ensure all eligible participants and their families obtain permanent housing by coordinating with private landlords, property management companies, etc.
  • Provide and coordinate resources required to reach housing stability goals.
  • Ensure all documentation, recertification, and contact requirements are met.
  • Develop individualized housing stability goals and sustainability plans in collaboration with program participants.
  • Conduct housing searches and inspections with program participants as needed.
  • Complete monthly home visits to housed participants.
  • Create realistic monthly budgets with program participants and plan for those with no income.
  • Request temporary financial assistance for participants as needed and complete documentation in a timely manner.
  • Maintain regular contact with program participants via bi-weekly phone or in-person contact and monthly in-person contact.
  • Conduct community outreach to increase program awareness and locate eligible individuals.
  • Distribute participant satisfaction surveys and register participants for the VA survey at the time of exit.
  • Transport Veteran families to potential housing options, resources, employment, and other needs if required.

Qualifications:

  • Bachelor’s degree in social work, healthcare, mental healthcare, healthcare administration, or a related field preferred.
  • At least 2 years of experience in healthcare or social services.
  • Veteran status, either currently active or previously discharged with anything other than a dishonorable discharge, is preferred.
  • Experience working with Veterans is highly preferred.
  • Valid Indiana Driver’s License and reliable transportation.
  • Strong interpersonal, oral, and written communication skills.
  • Proficiency with Microsoft Office applications, including Word, Teams, Outlook, Excel, and PowerPoint.
  • Proficiency with video conferencing platforms (e.g., Zoom, Microsoft Teams).
  • Proficiency with databases and the Homeless Management Information System (HMIS).
  • Excellent organizational and time management skills.

Working Conditions:

  • Hybrid position with 40% remote work and 60% community work.
  • Travel may be required up to 50%.
  • Ability to participate in video conferencing, in-person meetings, and phone conversations.
  • Occasionally lift and/or move up to 10 pounds.
