The Case Manager is responsible for providing confidential addiction guidance, education and support to individuals, families, and the community, as well as promoting healthy lifestyles and healthy choices. Under the supervision of the Director of Clinical Services, and in collaboration with a multi-disciplinary treatment team, the Case Manager ensures post treatment placement to the most appropriate level of care as recommended for each client. The goal of the case manager is to help clients re-enter society and re-engage in their regular lives as soon as possible. They may help clients accomplish this goal by coordinating care with external providers and community resources such as: Family Members, Primary Care Physicians, Psychiatrists, Intensive Outpatient, Partial Hospitalization, Legal Aids, Family Interventionists, University and High School Administrative Staff, and Employee Assistance Program Representatives. They may also help by assisting them with practical needs like finding housing, helping them find employment and arranging for continuing outpatient treatment after being discharged from the residence, if needed.
