Responsibilities

• Coordinate the multi-disciplinary activities related to maintaining continuity of care from the hospital to the home setting for Swing Bed, Acute, and Emergency Department Patients.
• Responsible for discharge planning, utilization review, team conferences, and unit-based rounds.
• Act as a liaison, performing outreach and rounds to hospital and providers to educate on and facilitate Swing Bed admission as a bridge to home.

Qualifications

• Minimum Education: Registered Nurse with New York State Licensure or Master’s Degree in Social Work (MSW) from an accredited School of Social Work with Licensure to practice in NYS or eligibility for licensure with valid permit; licensure expected within 12 months of employment.
• Minimum Experience: Demonstrated assessment skills. Ability to apply problem-solving theories and techniques with diverse populations. Strong time management, verbal and written skills, critical thinking, and organizational abilities. Knowledge of community resources, third-party payers, and regulatory agencies. Computer competency preferred.
• Required Certification/Registration: Case Management Certification within 36 months of employment is preferred.
• Physical Requirements: Light work; lifting up to 25 pounds, with occasional heavier lifting. 60% sitting and 40% standing or walking. May require bending, twisting, stooping, kneeling, and reaching. Ability to express ideas verbally.
• Working Conditions: On-site environment with typical facility safety and exposure controls. May include evenings or weekends for operational/community events as necessary.

• Salary: The compensation range for the role is $39.05 - $48.81 per hour. Garnet Health System provides a compensation range to comply with New York State salary transparency laws.
