Job summary

Azza HealthCare Agency seeks a dedicated care coordinator to coordinate care for inpatient and outpatient rehabilitation patients, managing a caseload from admission through discharge.

You will conduct comprehensive assessments, develop individualized care plans, coordinate with an interdisciplinary team, educate patients and families, arrange transitions to post-acute services, connect to community resources, and ensure continuity of care throughout the admission-to-discharge process.
