Job summary

Youth Opportunity & Osceola Youth Center in Evart, Michigan seeks a Case Manager to deliver clinical‑level case management for youth in care under supervision of a licensed professional. You will coordinate treatment activities, communicate with families and stakeholders, and help youths prepare for reintegration, discharge planning, and aftercare.

• Emotionally and physically able to engage in youth activities and program duties.

Responsibilities

• Provide case management services, including communication with youth, families, and Juvenile Probation Officers
• Complete or coordinate required mental health and substance abuse assessments
• Develop and update treatment plans, aftercare plans, performance summaries, and discharge summaries
• Support data collection for post-release tracking and outcomes
• Serve as liaison between the program, DJJ, families, and external agencies
• Prepare families for visitation, home leave, aftercare, and group/family meetings
• Connect youth and families to appropriate community resources
• Participate in internal audits and reviews to ensure policy compliance

Education

Pay: $45,000 - $49,920 per year based on experience
Schedule: Primarily weekdays with flexibility for program needs

About Youth Opportunity & Osceola Youth Center

Youth Opportunity is a nationally recognized leader in residential and behavioral health treatment for at‑risk youth. Osceola Youth Center in Evart, Michigan is a trauma‑informed Youth Center serving boys ages10 to17 requiring behavioral stabilization. The program merges clinical therapy, education, life skills training, and expressive recreation to support emotional growth in a structured setting.

Youth typically remain in care for 6 to9months, engaging in individual, family, and group therapy; restorative justice; victim‑impact awareness; and pre‑vocational preparation. Programming in small group units encouraging family partnership and successful return to the community.

About the Role

Preferred

• Two years of experience working with youth with emotional or substance use challenges
• Experience in juvenile justice, Medicaid-funded environments, or group facilitation
• Familiarity with treatment planning, case documentation, and family engagement strategies
