Job summary

Emporia Presbyterian Manor is seeking an RN Case Manager to supervise Medicare Part A and coordinate care for residents. The role ensures eligibility determination, paperwork completion, and seamless care during Medicare stays.

We offer excellent benefits and a premium pay plan with starting wages at $35 per hour plus shift and weekend differentials, with additional pay based on experience.

Qualifications

• Must possess a Nursing Degree from an accredited college or university or be a graduate of an approved RN program.
• Must possess a current active license to practice as a RN in this state.
• Prefer at least two years of experience in a hospital or long-term care community with experience in rehabilitative and restorative nursing practices and Medicare procedures.

Responsibilities

• Supervise Medicare Part A and facilitate managed care programs.
• Determine resident Medicare eligibility and ensure paperwork is completed during the stay.
• Monitor and coordinate care during Medicare stays in accordance with guidelines.
• Support the Health Services Director, Health Services Administrator or Executive Director to ensure high-quality care.

TheRegistered Nurse (RN) Case Manageris responsible for supervising Medicare Part A and facilitating managed care programs by determining whether a resident is eligible for Medicare, ensuring paperwork is completed during the resident’s stay and monitoring and coordinating care while a resident is in a Medicare stay. Such supervision must be in accordance with federal and state guidelines and regulations and as may be required by theHealth Services Director, Health Services Administrator or Executive Director to ensure the highest degree of quality care be provided. Advances the value that the resident comes first. The employee values the Community as the residents’ home and works to create attributes of home and models person centered care centered care.
