Job summary

Damar Services is seeking a dedicated Case Manager in East Chicago, IN to lead treatment services for individuals with DD/ID or autism and to promote the agency philosophy in care delivery.

The role requires 1+ year of relevant experience or specialized training, extensive IPP development, and collaboration with residential teams. Strong documentation and family engagement skills are essential.

Qualifications

  • At least one year of experience in treating DD/ID population or specialized training.
  • Be at least 21 years old per licensure requirements.
  • Valid Indiana driver's license and driving record that meets eligibility requirements of Damar's insurance carrier (for all positions requiring driving).
  • No felony convictions, substantiated charges of neglect or other disqualifying criminal convictions.
  • Successfully complete a pre-employment drug screen and physical.
  • Favorably meet and maintain Damar’s background screening.

Responsibilities

  • Lead, direct and monitor treatment services.
  • Promote agency philosophy in the delivery of services.
  • Represent clients at doctor’s appointments and exchange information effectively and accurately.
  • Maintain service notes, contacts, data sheet creation and data collection.
  • Plan and lead Program meetings.
  • Develop IPPs for each assigned client within 7 days of admission and update as needed.
  • Collaborate with Residential Managers to ensure execution of unit programming.
  • Lead IPP implementation by providing guidance and training to employees for assigned clients.
  • Write and update monthly IPP to ensure they are ready by the 6th of every month.
  • Review and sign MHA and update the addendum as needed.
  • Complete an ICAP assessment within 30 days of admission.
  • Complete Ansell Casey assessments (within 20 days from admit and update annually).
  • Monitor the implementation and documentation of IL goals and complete IL monthly reports.
  • Ensure Family engagement communication (weekly) per client in a caseload and referrals source communication (weekly).
  • Attend CFTM Quarterly or as needed and represent clients in Court Reviews and IEP/Case Conferences with accurate information.

Education

Bachelor’s Degree in education from an accredited program
Bachelor’s Degree or Master’s Degree in social work from an accredited program
Bachelor’s or Master’s Degree in a human services field (other than social work) and at least three (3) years of experience under the supervision of a qualified social worker OR at least one year experience working directly with persons with DD/ID
Registered Nurse
Therapeutic recreation specialist who is a graduate of an accredited program
Master’s degree in psychology from an accredited program

  • Independent Living skills

What you need to be considered:

Essential Job Functions

The essential functions of this position require the physical ability and mobility needed to perform the job.
