Job description

Peer Assistance Services, Inc. is a community-based, 501(c)(3), non‑for‑profit Colorado corporation. Our mission—to lead with prevention and intervention for substance use and mental health concerns—guides all agency programming. PAS implements two statewide programs, Screening Brief Intervention, Referral to treatment (SBIRT), and the Peer Health Assistance Programs (PHAP).

About the Case Manager Position

This is a hands‑on, full‑time position with day‑to‑day responsibilities that will include ensuring program participants are able to practice their profession with reasonable skill and safety, while complying with regulatory requirements. You will also collaborate with licensed evaluators to develop a monitoring contract outlining treatment and community resources to optimize program participants' physical and mental health and well‑being. You will maintain regular communication with program participants, treatment providers, and workplace supervisors to measure compliance with the monitoring contract and may adjust contract requirements based on compliance measurements while keeping accurate records throughout the participant’s involvement in the Peer Health Assistance Program. Your goal is to help program participants achieve optimal health and well‑being while also meeting regulatory requirements.

Compensation

$57,000–$62,000

Essential Duties
  • Provide comprehensive case management for Peer Health Assistance Program participants.
  • Determine monitoring contract requirements in collaboration with clinical evaluators.
  • Provide referrals to community resources and all levels of mental health or substance use treatment; communicate with providers to ensure comprehensive care.
  • Monitor compliance through correspondence with treatment providers and employers, verification of treatment and support group attendance, and review of drug testing results.
  • Review all incoming reports, screening tools, and correspondence for accuracy and compliance; manage a high volume of documentation and identify concerns requiring follow‑up or clinical escalation.
  • Approve potential job opportunities to ensure alignment with program requirements and safety to practice; develop practice agreements that outline employment modifications and reporting expectations.
  • Modify monitoring contracts based on documented compliance, recommendations from providers, return to substance use, or recurrence of mental health symptoms.
  • Conduct regular check‑ins with each participant on a case load.
  • Prepare discharge requirements and documentation.
  • Maintain regular communication with regulatory boards/programs regarding participant compliance, status updates, and program requirements.
  • Present complex cases during interdisciplinary case consultations, document internal case staffing discussions, clinical recommendations, and compliance considerations, and ensure timely follow‑through on agreed‑upon action steps.
  • Prepare quarterly compliance reports.
  • Participate in program outreach by staffing exhibit tables, providing presentations, and attending events for partner organizations.
  • Establish and manage relationships with key partners, e.g., treatment and healthcare providers, employers, supervisors, and practice monitors.
  • Attend staff meetings including case consultation, office hours, case review, required training, and all agency meetings.
  • Assist with additional program activities as requested.
What education & experience is required in this role?
  • Bachelor’s degree in a health or behavioral health related field is required.
  • Two or more years of experience in case management is required.
  • Experience in community mental health settings such as forensic or competency‑related services is preferred.
  • Strong skills in structured monitoring, compliance tracking, and participant accountability.
