Job summary

Cornerstone Community Center in Wisconsin is seeking a full-time, in-person Community Case Manager to assess the needs of older adults and connect them with local services in DeForest, Windsor, and Vienna.

The ideal candidate has experience with older adults, knowledge of aging issues, and familiarity with case-management standards. Strong written and verbal communication, solid computer and organizational skills, and a valid driver’s license are required.

Qualifications

• Experience working with older adults and knowledge of aging issues and community resources.
• Familiarity with case management standards and professional conduct.
• Good verbal and writing skills and ability to follow oral/written instructions.
• Computer and organizational skills for documenting, planning, monitoring, and evaluating senior services.
• Valid driver’s license, access to transportation and ability to pass a background check.

The position is a full-time position. The pay range is based on experience, knowledge and ability of the individual selected for this position. Health and Dental insurance, sick leave, vacation, and WRS retirement are included. $24.00-$28.00/hour Equal Opportunity Employer.
