Job Summary

Coordinates the delivery of rehabilitation services to patients in collaboration with facility staff. Monitors and documents the cost effectiveness of treatments, facilitates admission and discharge processes, and serves as a patient advocate and liaison to insurance carriers and other medical management professionals.

Qualifications
  • Current California LVN or RN license in good standing
  • Minimum 3 years experience in a hospital, nursing, or related healthcare facility
  • Working knowledge of managed care rules, regulations, and guidelines
  • Outstanding communication skills (oral and written)
  • Ability to multi-task and treat everyone with kindness and professionalism
  • Works collaboratively and independently, following procedures and guidelines
  • Completes tasks on time, reports successes and challenges, and offers solutions
  • Emotional maturity – ability to remain calm and professional under stress
Responsibilities
  • Greet new patients/residents to ensure a positive admission experience
  • Evaluate and communicate resident status, changes in functions, and care plans to payers
  • Document all payer interactions regarding resident progress, outcomes, and instructions
  • Meet with interdisciplinary team to coordinate services and ensure comprehensive care
  • Coordinate and communicate with business office and medical records to ensure accurate census and payment of managed care and Medicare residents
