Liberty Resources in Madison County seeks a full-time Residential Case Manager to coordinate housing services for Venture House residents and the Apartment Treatment program. Qualifications

• Bachelor's or Associate's degree OR HS Diploma with 3 years related experience.
• Valid driver's license and reliable transportation.
• Experience coordinating housing related services and supports.

Responsibilities

• Completes assessment of housing and housing related needs.
• Works with Venture House and Apartment Treatment staff to establish individualized service plans.
• Establishes and maintains relationships with other service providers (DSS, SPOA, PROS, etc.).
• Acts as program liaison for client coordination of services.
• Demonstrates understanding of mental health regulations and related treatments.
• Assists individuals with linkage to substance use and mental health services.
• Responds to emergencies and adjusts time/area of assignment as needed.
• Completes required documentation in a timely manner per program standards.

Residential Case Manager Position Summary:

The Case Manager performs case management-related functions to coordinate, facilitate and maintain care for the residents of Venture House and the Apartment Treatment program in Madison County.

Residential Case Manager Responsibilities:

• Completes assessment of housing and housing related needs.
• Works with Venture House and Apartment Treatment staff, and multi-disciplinary treatment team to establish individualized service plans, which meet the clients’ goals.
• Establishes and maintains effective relationships with other service providers: e.g. Madison County DSS, SPOA Coordinator, PROS, Care Coordination, etc.
• Acts as program liaison and primary contact for other service providers as it relates to clients coordination of services.
• Demonstrates current understanding of the Office of Mental Health regulations, mental health disorders and its treatments. Understands reciprocal relationships between substance abuse and mental health.
• Assists individuals with linkage and referral to substance use disorder and mental health services where necessary.
• Responds to and effectively handles emergencies, which may require flexibility of time and area of assignment.
• Completes all required documentation, in timelines in accordance with program standards.

Residential Case Manager Qualifications:

Bachelors/Associates and one (1) year experience or HS Diploma and three (3) years related experience. Must have a valid driver's license and access to reliable transportation.

Pay Rate:

$21.57 per hour
