Under general supervision, is responsible for the development and implementation of the Individualized Service Plans for individuals and their advocates in accordance with the regulations and requirements of the applicable governmental and other regulatory agencies within a residential setting. Provides case management services to assigned individuals and determines individual aptitudes, interests and preferences. Identifies appropriate service providers and links individuals with providers. Coordinates services and monitors individual’s satisfaction with the services.
