The BridgeLine, Inc. is seeking a full-time Case Manager to provide comprehensive case management for adults with brain injuries in Charlottesville and 13 surrounding counties.

You will assess needs, coordinate services, develop service plans, monitor progress, advocate for clients, and educate families and the community. Qualified candidates will have a Bachelor’s degree in a related field, two years of experience, and a valid Virginia driver’s license with a clean DMV record, along with strong

Responsibilities

• Provide case management services to adults with brain injuries in Charlottesville and 13 surrounding counties.
• Perform assessments of service needs and coordinate services among providers.
• Develop and monitor individualized service plans; advocate and educate clients, families, and the community.
• Coordinate care across multidisciplinary teams and community resources.
• Document progress and communicate with stakeholders to ensure quality outcomes.

Job description

The BridgeLine is accepting applications for a full-time Case Manager position. This position provides case management services to adults with brain injuries in Charlottesville and 13 surrounding counties. The incumbent will provide an assessment of service needs, coordination of services among providers, development of service plans, monitoring of needs, advocacy, consultation, and education for clients, families, and the community.
