Job summary

Vista del Mar Hospital is seeking a dedicated Case Manager to join our multidisciplinary team. You will assess client needs, develop individualized discharge plans, and coordinate services to ensure safe housing, transportation, and aftercare arrangements.

The role involves direct counseling for addiction and behavioral health concerns, and you will relay information to families with sensitivity and clarity.
