Father Bill’s & MainSpring is seeking a Case Manager in the Housing Department to provide community-based support to individuals and families experiencing homelessness. The role uses a housing-first approach, with assessments, planning, and ongoing evaluation of needs.

Willingness to transport clients in a personal vehicle and conduct home visits is required. Bachelor’s Degree or relevant experience is required; a Master’s Degree and/or 2+ years direct care experience is preferred.

Responsibilities

  • Provide outreach and assessment in the community as deemed necessary by supervisor or administrator.
  • Evaluate and assess referrals using a housing-first framework and identify barriers to housing.
  • Collaborate with shelter staff to transition guests from shelter to permanent housing.
  • Collect and maintain data-based caserecords and timelines.
  • Create and maintain accurate case files and required documentation for funders.
  • Develop participant-centered individual service plans within the required timeframes.
  • Monitor progress and complete assessments to identify strengths, needs, barriers and goals.

We help people who are struggling with homelessness or are at risk of homelessness to achieve self-sufficiency.

Father Bill’s & MainSpring is a growing and dynamic social service agency with particular expertise in emergency and permanent housing, job training and placement, and prevention services for individuals and families in need. Over the next five years, our strategic focus includes an increase in affordable, permanent supportive housing, a more efficient model of care for families experiencing homelessness, commitment to an enhanced delivery model to reduce the need for shelter, and improved community relations in order to achieve these goals. Position Summary:

Housing Case Managers provide community-based support to individuals and/or families who have experienced homelessness. Case Managers work with people in their homes, utilizing a housing first approach, understanding people who have experienced chronic homelessness require additional support to achieve positive tenancies. This full- time position provides on-going case management support and expertise through comprehensive assessment, planning, implementation, and overall evaluation of participant needs. Willingness to transport clients and their belongings in personal vehicle as well as meeting with clients through community outreach. Bachelor’s Degree and/or relevant experience is required, Master’s Degree in social work or related field preferred and/or 2+ years direct care experience in related field.

Essential Responsibilities:
  • Provideoutreachandassessmentinthecommunityasdeemednecessarybysupervisororotheradministrator within theorganization.
  • Evaluationandassessmentofreferrals,utilizingahousingfirstframeworkandunderstandingthebarriersto house people experiencinghomelessness.
  • Collaborate with FBMS shelter staff to foster a smooth transition for guests moving out of shelter and into permanent supportivehousing.
  • Collectandmaintaindata-basedstatisticalinformation,maintainaccuratecaserecordsinatimelyfashion.
  • Setupandmaintainaccuratecasefiles aswellasadheretorequirementsasdeterminedbyprogramfunders.
  • Create and implement participant-centered individual service plans within required time frame with an emphasis on participantgoals.
  • Monitorparticipantprogressandcompleteassessmentsinordertoidentifyparticipant’sstrengths,needs,

barriers and goals in order to sustain housing and increase self-sufficiency.

  • Educate participants on activities of daily living as necessary and refer to providers asneeded.
  • Complete home visits as well as assist participants to access community resources, schedule and attend appointments (some transporting isrequired).
  • Locateandsecurehousingopportunitiesforparticipantsoftenwithinashortwindowof identifiedtime.
  • Network and maintain positive working relationships with local landlords, property managers, housing authorities, social service organizations and stateagencies.
  • Provide support, facilitate crisis intervention, mediation and service coordination whennecessary.
  • Collaborate with internal and externalpartners.
  • Participate in regularly scheduled supervision, agency and community meetings and trainings whenrequired.
