Job summary

Casa Esperanza, Inc. is seeking a dedicated Case Manager to provide comprehensive case management and care coordination for patients and their children in the LyN Residential Treatment Program.

You will coordinate medical, behavioral health, social services, legal and community resources, with a focus on ensuring continuity of care and aligning with Casa Esperanza’s mission and values. Onsite schedule is Monday–Friday, 8am–4pm, reporting to Clinical Supervisor.

Qualifications

  • Two years of experience working in social services with a substance abuse population.
  • Certified Alcohol and Drug Counselor (CADC) or willingness to obtain within 1 year.
  • Bilingual Spanish/English required with cultural knowledge.
  • CPR certification is required.

Responsibilities

  • Provide ongoing case management and care coordination for patients and their children.
  • Develop and monitor patient-centered service plans with the treatment team.
  • Coordinate medical, behavioral health, dental, psychiatric, and appointments.
  • Collaborate with internal and external providers for timely access to services.
  • Manage DCF-related responsibilities and reunification planning.
  • Refer patients to housing, employment, childcare, and community resources.
  • Assist patients with legal obligations and court-related appointments.
  • Facilitate psychoeducational and recovery-focused groups.
  • Participate in team meetings and treatment planning.
  • Maintain accurate documentation in the electronic health record.
  • Provide crisis intervention and patient advocacy.

About the role

The Case Manager provides comprehensive case management and care coordination services for patients and their children enrolled in the LyN Residential Treatment Program. This position supports patients in achieving treatment and recovery goals by coordinating medical, behavioral health, social services, legal, and community resources while ensuring continuity of care and promoting the mission and values of Casa Esperanza.

What you'll do
  • Provide ongoing case management and care coordination for patients and their children.
  • Develop and monitor patient-centered service plans in collaboration with the multidisciplinary treatment team.
  • Coordinate medical, behavioral health, dental, psychiatric, and specialty appointments, transportation, and medication refills.
  • Collaborate with internal departments and external providers to ensure timely access to services.
  • Manage DCF-related responsibilities, including communication, reunification planning, toxicology reporting, child visitation, and collateral contacts.
  • Coordinate referrals to community resources, including housing, DTA, WIC, employment, childcare, parenting services, domestic violence resources, and aftercare support.
  • Assist patients in meeting legal obligations, including court, probation, parole, and other required appointments.
  • Facilitate psychoeducational and recovery-focused groups.
  • Participate in multidisciplinary team meetings, case conferences, supervision, and treatment planning.
  • Maintain accurate and timely documentation in the electronic health record, including progress notes, discharge planning, and case closure documentation.
  • Provide crisis intervention, patient advocacy, and problem-solving to address barriers to treatment and recovery.
  • Maintain confidentiality and comply with all agencies, state, and federal regulations.
  • Perform other duties as assigned.
Qualifications
  • Skills necessary for the position:
  • Ability to work as a team member

  • Knowledge of the substance use field, including the AA/NA 12-step philosophy
  • Knowledge of appropriate community resources
  • Strong verbal and written communication skills
  • Ability to communicate with clients and referral sources in a professional manner
  • Strong interpersonal skills and customer service skills
  • Knowledge of computer software, including Microsoft Word and Excel
  • Valid Massachusetts Driver’s License
  • Ability to meet the physical and training requirements of the position.
Experience necessary for the position:
  • Experience working in an advocacy role for children and families
  • Experience with homelessness, mental illness, domestic violence, HIV/AIDS, and child case management
  • Experience with motivational interviewing and trauma-informed care, preferred.
Education/Training required:
  • Two years of experience working in social services with a substance abuse population
  • Certified Alcohol and Drug Counselor/Certified Alcohol Counselor required or willingness to obtain within 1 year of hire.
  • Bilingual Spanish/English required with intimate knowledge of the cultural, social, economic, health, and substance abuse factors.
  • CPR Certified

  • Compensation: $25 - $26/hour
  • 401(k) Retirement Plan: All eligible employees may elect to participate after 1 year of service. Casa Esperanza, Inc. will match a contribution of up to 6% of your annual salary
