Job summary

Spectrum Health Systems, Inc. is seeking a Case Manager to provide documented individual and group clinical services and case management for clients with mental health, substance abuse, or dual diagnosis issues at Westborough, MA.

The role supports treatment planning, referrals, and multidisciplinary team participation, with a Tuesday-Saturday schedule and a $21.00 per hour pay rate, plus a comprehensive benefits package.

Qualifications

• Experience in substance abuse treatment or related field.
• LADC I or CADAC certification preferred, but not required.
• Minimum of a high school diploma, but Bachelor's Degree preferred.

Responsibilities

• Providing documented individual and group clinical services and case management to clients with mental health, substance abuse, or dual diagnosis problems.
• Developing, implementing and revising individual treatment plans for assigned clients in established time frames with client participation.
• Referring clients to appropriate services in the community as necessary.
• Participating in multidisciplinary team treatment planning, case review and case conferences.

Schedule: Tuesday-Saturday 8:00am-4:30pm and one shift of 10:00am-6:30pm

Pay rate: $21.00/hour

• Providing documented individual and group clinical services and case management to clients with mental health, substance abuse, or dual diagnosis problems.
• Developing implements and revising individual treatment plans for assigned clients in established time frames with the participation of the client.
• Referring clients to appropriate services in the community as necessary.
• Participating in multidisciplinary team treatment planning, case review and case conferences.

Qualifications

Qualifications:

• At least one year experience working in a substance abuse treatment, case management role or closely related field
• Minimum of a high school diploma, but Bachelor's Degree Preferred
