Firefly Children & Family Alliance seeks a dedicated Home-based Case Manager in Muncie, Indiana. The role will travel across counties to support families in achieving their goals, with field-based duties and some evenings.

You will conduct intake, provide education and transportation, attend court with clients, and coordinate with schools and social services to stabilize family functioning.

Qualifications

• Bachelor's degree in social work or related field is required.
• Must be at least 21 years of age.
• A valid driver's license and insurance with no more than 6 points on driving record.
• Travel between appointments to service clients within their community.
• Knowledgeable regarding social service systems within the state.
• Knowledge of and adherence to current confidentiality practices.
• Appreciation for and ability to respond to cultural differences.

Responsibilities

• Provide home-based services to families to assist in achieving goals.
• Visit youth/families in their homes on a routine and pop-in basis.
• Assist families in achieving goals and providing education/support.
• Provide transportation to youth/families.
• Attend court with clients and render recommendations.
• Communicate with schools and multiple institutions related to youth/family matters.
• Respond to crisis calls and manage crises.
• Create and update plans and goals for each youth/family.
• Develop relationships with community agencies and other social welfare organizations.

Firefly Children & Family Alliance is looking for a full‑time Home‑based Case Manager in Muncie, Indiana. This position is primarily in Muncie, but could cover Wells, Adams, Grant, Blackford, Jay, Delaware, and Randolph counties as needed. Case Managers provide services to families in the families' homes on a routine basis to assist them in obtaining their goals. You will need to provide education and support to these families and transportation. You will support your clients by attending court, responding to crisis and stabilizing family functioning. This position is field‑based and will primarily work out in the community, with some evenings required.

You Will:

(Family Progress - Monitoring and Coaching (50%)

Coach collaboratively - maintaining a balance between teaching and serving

Visit each youth/family on a routine and 'pop in' schedule

Assist youth/families in achieving their goals

Provide education and support to youth/families

Provide transportation to youth/families

Attend court with clients and render recommendations

Communicate with schools and multiple institutions related to youth/family matters

Respond to all crisis calls and manage those situations

Create plans, including goals, for each youth/family

Update plans and goals

Develop relationships with agencies in the community

Work with other social welfare agencies

Intake and Needs Assessment (15%)

Determine 'crisis' situations

Visit each youth/family to gather necessary information to complete the assessment

Follow Firefly Children & Family Alliance professional standards

Report (15%)

Maintain case and agency paperwork according to standards

Prepare written reports for the Department of Child Services / Probation office within identified timeframes

You Have:

• Bachelor's degree in social work or related field is required.
• Must be at least 21 years of age
