Job summary

Anew is seeking a Behavioral Health Case Manager to provide comprehensive support and coordination of care for clients facing mental health challenges. This role involves assessing client needs, developing individualized service plans, and collaborating with multidisciplinary teams to ensure delivery of effective and client-centered services.

The Case Manager helps facilitate recovery, promote well-being, and support clients toward independence, while navigating community resources and ensuring

Qualifications

• Strong interpersonal and communication skills with clients, families, and professionals.
• Knowledge of behavioral health practices and community resources.
• Ability to work independently and as part of a team.
• CPR certification preferred; NPI number/Ohio Medicaid ID is required.
• Minimum of 3 years in behavioral health or related field preferred.
• Background checks per company policy.

Responsibilities

• Coordinate and provide care that is safe, timely, effective, efficient, and client-centered.
• Manage case assignments, draft service plans, and review case progress regularly.
• Advocate for clients, assisting them in making informed decisions regarding their clinical status and treatment options.
• Collaborate with therapists, clients, and client family members to ensure all client behavioral health needs are met.
• Facilitate clients in achieving personal independence and assist in developing daily living skills.
• Perform other duties as assigned.
• Upholds Documentation and Billing practices associated with CARF, CMS, and as training indicates.

Supervision Given:

This position does not supervise any staff.

Supervision Received:

The Behavioral Health Case Manager reports to their assigned Operations Managers.

• Knowledge of behavioral health practices and community resources.
• Ability to work independently and as part of a team.

Education:

• High school diploma or equivalent is required;
• a college degree in Human Services is preferred.

Certification:

• CPR certification is preferred.
• NPI number/Ohio Medicaid ID is required.

Experience:

• Minimum of 3 years in behavioral health or a related field is preferred, but not mandatory.

Physical Effort:

• Requires sitting for extended periods and occasional lifting of up to 20 pounds.

Background:

• Must pass all Federal and State background checks according to company policy.
