• CPR certification is preferred.
• NPI number/Ohio Medicaid ID is required (instructions on how to apply will be provided).

Responsibilities

• Coordinate and provide care athat is safe, timely, effective, efficient, and client-centered.
• Manage case assignments, draft service plans, and review case progress regularly.
• Advocate for clients, assisting them in making informed decisions regarding their clinical status and treatment options.
• Collaborate with therapists, clients, and client family members to ensure all client behavioral health needs are met.
• Facilitate clients in achieving personal independence and assist in developing daily living skills.
• Perform other duties as assigned.
• Upholds Documentation and Billing practices associated with CARF, CMS, and as training indicates.

Summary

The Case Manager is dedicated to facilitating clients' recovery and promoting their overall well-being and independence.

Supervision Given:

This position does not supervise any staff.

Supervision Received:

The Behavioral Health Case Manager reports to their assigned Operations Managers.

Physical Effort
