Job Summary :

Interviews, accepts, and provides comprehensive, long-term, structured, complex case management services for an assigned caseload of clients participating in an established life management program. Understands the uniqueness of the client’s history to determine the most effective program plans. Develops comprehensive program plans/goals and evaluates client progress through mentoring and counseling sessions with clients and/or staff. Serves as an advocate for clients to acquire services that enable them to functionally cope with their environment. Ensures constant compliance with funding requirements.

Essential Functions :

Case Management Responsibilities (40%)
  • Establishes long-term program goals based on client assessments; explains goals clearly to clients; counsels clients on modifying negative behaviors.
  • Outsources clients to additional counseling resources as needed; maintains awareness of external case management progress.
  • Meets regularly with clients to discuss progress; documents sessions; enforces house guidelines disciplinary actions.
  • Facilitates discussions to promote positive attitudes and motivation; redirects negativity; encourages self-awareness.
  • Plans and facilitates life management, behavior modification, and addiction classes; arranges speakers and workshops.
  • Maintains awareness of program funding requirements; ensures outcomes meet funding standards; adjusts processes as needed.
  • Analyzes statistical data to prevent recidivism; recommends program changes to reduce service requests.
  • Completes discharge plans; attends community meetings to educate and refer clients.
Social Services Responsibilities (35%)
  • Greets and interviews clients; determines eligibility; records pertinent information; assists with applications.
  • Prepares and processes vouchers; directs clients to resources.
  • Locates and arranges housing; contacts shelters and lodges.
  • Processes financial assistance; contacts utility organizations and landlords.
  • Assists with transportation needs; liaises with other agencies; builds community relationships.
  • Conducts needs assessments; assists with school and childcare enrollment; helps locate housing and supports clients’ economic stability.
Food Pantry Responsibilities (15%)
  • Prepares food packages; sorts donations; manages storage and distribution.
  • Maintains inventory; ensures food safety; supervises community service workers.
Administrative Responsibilities (10%)
  • Maintains case records and logs; prepares statistical reports; presents cases as needed.
  • Handles recordkeeping for expenditures; performs other administrative duties.
Other Responsibilities
  • Conducts facility tours; assists in policy manual development; supports community services and seasonal projects.
Materials and Equipment :

Knowledge, Skills, and Abilities :

Knowledge of social service principles, community resources, effective communication, and outcome measurement. Ability to develop plans, evaluate progress, work with diverse populations, maintain records, and build professional relationships.

Minimum Qualifications :

Education: Bachelor’s degree in Social Work, Behavioral Science, or related field. Experience: Three years of direct case management social services. Or equivalent training and experience.

Certifications: Valid State Driver’s License.

Physical Requirements :

Attendance, effective communication, quick thinking, multi-tasking, light physical effort, seated work.

Working Conditions :

Performed in shelter environments; may involve exposure to health risks, unsanitary conditions, weather, noise, dust, dirt; includes driving for off-site visits.
