Job summary

IU Health LifeCare at Methodist Hospital seeks a Case Management Specialist to support outpatient HIV treatment and prevention services. You will be part of the Social Work team, coordinating care and resources for patients in a multidisciplinary clinical setting.

This role requires a high school diploma (associate's preferred), 1–3 years of experience, and strong computer skills for entering progress notes in medical records.

Qualifications

• 1–3 years of experience required.
• Ability to understand third party contractual arrangements.
• Basic computer skills to enter notes in multiple medical record systems; familiarity with Excel/Word/Outlook/PowerPoint.
• Knowledge of anatomy, physiology and medical terminology.
• Knowledge of managed care and government healthcare benefits.

Responsibilities

• Provide case management to LifeCare patients within a multidisciplinary clinical setting.
• Document progress notes and manage data in medical record systems.
• Collaborate with the Social Work team and clinicians to coordinate care.
• Assist patients with care coordination and benefits navigation.

Position Details:

Status: Full-Time, 40 hours per week.

Schedule: Mondays: 9:30am - 6:00 pm. Tuesdays through Fridays: 8am - 4:30pm.

Position Summary:

LifeCare provides outpatient adult HIV treatment and prevention services in a multidisciplinary clinical setting located at Methodist Hospital. The Case Management Specialist is part of the Social Work team that supports the patients of LifeCare and is an integral part of the health care team.

Position Requirements:

• High School Diploma/GED required. Associate's Degree preferred.
• 1-3 years of experience required.
• Requires the ability to comprehend third party contractual arrangements.
• Requires basic computer skills including the ability to enter progress notes and other data in multiple medical record systems. Basic familiarity with Excel, Word, Outlook, and PowerPoint.
• Requires some knowledge of anatomy, physiology, and medical terminology.
• Requires some knowledge of managed care and government healthcare benefits.
