• Counsel patients and families with discretion in emotionally charged situations.
• Communicate effectively in writing and verbally; use modern technology.

Responsibilities

• Conduct assessments and provide psychosocial interventions with patients and families.
• Provide case management across the continuum of care, including discharge planning.
• Record assessment and progress in the medical record.
