Requirements include RN license in New York, BSN preferred, PRI certification within 6 months, and 1 year of acute care experience. Case management experience is preferred.

The RN Case Manager duties and responsibilities help patients who have multiple health conditions, difficult to control chronic conditions, disabilities and other challenges, get care they need. Through partnership and collaboration with all members of the healthcare team, the Case Manager is the principal liaison between emergency services, surgical services, nursing, physicians, utilization review, and medical records. In addition, they are responsible for coordinating safe and appropriate discharge plans for the patients within their service area.
