Community Case Manager

Brewer, ME (Remote with Travel) | Flexible Schedule | Independent Contractor

Morrison Center is growing—and we’re looking for dedicated, organized, and compassionate Case Managers to join our expanding Adult Case Management program in the Brewer, ME area.

This is a remote contract position that offers the best of both worlds: the flexibility of working from home, with the rewarding, hands-on engagement of in-person home and community visits.

️ Key Responsibilities:

  • Support adults with intellectual and developmental disabilities in achieving their personal goals
  • Develop and manage Person-Centered Plans (PCPs), assessments, and service goals
  • Ensure all documentation is accurate and compliant with Department of Health and Human Servicesrequirements
  • Conduct monthly home visits and maintain open communication with clients and families
  • Perform research and collaborate with service providers to ensure effective supports are in place
  • Serve as a trusted resource and advocate in the community

Qualifications:

  • Bachelor’s degree in any field (required)
  • Minimum 1 year of experience working with children or adults in human services
  • Experience with PCPs, family systems, and the Evergreen system is preferred
  • Strong communication, time management, and problem-solving skills
  • Ability to work independently with a high level of professionalism

Contract Details:

  • Contract Type: Independent Contractor
  • Pay: $65/hour
  • Contract Duration: Rolling, reviewed annually
  • Notice Period: 30 days’ notice required for termination
  • Schedule: Flexible hours (Monday–Friday)
  • Location: Remote with travel for home/community visits in the Brewer area
