Job description

Morrison Center is looking for Case Managers to provide Adult Case Management services in the Westbrook, ME area, in response to our expanding program. This is a remote rote, requiring traveling for home and community visits.

Scope of Responsibilities:

  • Assist clients in achieving their goals by completing necessary tasks and requirements.
  • Maintain documentation in compliance with regulatory agencies, such as the Department of Health and Human Services.
  • Conduct monthly home visits.
  • Develop and manage Person-Centered Plans (PCPs), assessments, and goals.
  • Maintain effective communication and perform research as needed.

Qualifications:

  • Minimum of 1 year of experience working with children or adults.
  • Bachelor’s degree in any field.
  • Experience with families and familiarity with PCPs and the Evergreen system is preferred.

Contract Details:

  • Duration: This is a rolling contract that will be reviewed annually.
  • Notice Period: Thirty days are required for termination of the contract.
  • Hours: Flexible.
