Here’s what you’ll be doing:

Coordinate care and resources for members across the care continuum by conducting comprehensive assessments and developing individualized, member-centric care plans.
Monitor member progress, provide education, and collaborate with providers, community resources, and internal partners to ensure optimal clinical and resource outcomes.
Document all aspects of case management including assessment, plan development, interventions, and outcomes, while meeting compliance and quality standards.
Maintain active RN licensure (NC or compact) and obtain any required specialty certifications (e.g., CCM, CDCES) within two years of hire, if applicable.

Here’s what you’ll have:
