Role Description

The RN Home Health and Hospice Case Manager, on-site role based in Salt Lake, UT. This role is responsible for coordinating and overseeing patient care plans in the home health and hospice setting, ensuring services are clinically appropriate and aligned with patient and family goals. Daily tasks include assessing patient needs, updating care plans, collaborating with physicians and interdisciplinary team members, and arranging services and resources to support safe transitions of care. The Case Management Nurse will monitor patient progress, provide education to patients and caregivers, and advocate for quality, cost-effective care. This role also involves timely documentation, participation in case conferences, and adherence to regulatory and agency standards.

Qualifications

• Candidates should possess strong nursing skills relevant to home health and hospice care.
• Candidates should possess Case Management and Utilization Management skills to coordinate care and optimize resources.
• Candidates should possess Discharge Planning skills to support safe transitions between care settings.
• Active Registered Nurse (RN) license in the state of Utah, in good standing.
• Previous experience in home health, hospice, or acute care case management is preferred.
• Excellent communication, interpersonal, and patient advocacy skills.
• Strong organizational skills with the ability to manage multiple cases and prioritize effectively.
• Ability to work collaboratively within an interdisciplinary team and with families from diverse backgrounds.
• Proficiency in electronic health records and basic computer applications.
• Valid driver's license and reliable transportation for patient visits, if required.
