Live Well Homecare Agency, LLC is seeking an experienced Nurse Case Manager Supervisor to oversee day-to-day case management and discharge planning, ensuring compliance with regulations and quality patient care.

The role requires leadership, strong communication, and expertise in clinical criteria, payer guidelines, and health information systems such as InterQual and Milliman guidelines.

Competenze

• Minimum three years of case management experience.
• Two years of leadership in case management.
• RN or BSN required; Case Management certification preferred.
• Strong critical thinking and communication skills; knowledge of clinical criteria and regulatory requirements.
