Job summary

The ideal candidate will have a bachelor’s degree in Nursing with RN licensure and 3 years of hospital-based Case Management experience. Responsibilities include managing a team of social workers, overseeing a caseload of patients, and ensuring effective case management operations.

Qualifications

• 3 years of hospital-based Case Management experience, including Utilization Review and Discharge Planning.

Responsibilities

• Manage a team of direct reports including full-time and part-time staff.
• Oversee a caseload of 11-14 patients.
• Carry a caseload when needed while handling front line responsibilities.

Smaller community healthcare facility part of a system looking to bring on Case Management Director! Sign On Bonus and Full Relocation!

Responsibilities

• Direct reports include 4 full-time staff and numerous prn staff
• Team consist of all Social Workers
• Typical case load expected of the Case Managers is 11-14 patients
• Director will carry a caseload as needed but should expect weekly to have front line responsibilities.

Qualifications

• Degree required, must possess bachelor’s degree in Nursing (BSN) with RN licensure
• Degreed required in SW, OT, PT, ST, or Rehabilitation Counseling.
• 3 years of hospital-based Case Management experience including Utilization Review and Discharge Planning
