Job summary

Griffin Health is seeking an experienced Registered Nurse in case management to gather data on patient treatment status, identify barriers, and help set realistic goals and alternatives to care.

The role collaborates with patient/family, primary care physician, and a multi-disciplinary team to develop a care plan that enhances outcomes and empowers patients in their own care.

Qualifications

• RN license required; BSN preferred.
• Two to three years hospital experience required; Home Care and/or Utilization Review experience desired.

MAIN FUNCTION: Gathers all relevant data related to patient’s treatment status; objectively evaluates it to identifies barriers, clarifies realistic goals & seeks potential alternatives to care. Works with the patient/family, primary care physician & multi-disciplines to develop a treatment plan that enhances patient outcomes; facilitates communication & collaboration between the patient & health team; advocates patient empowerment in their own care.

EDUCATION: RN required, BSN preferred

EXPERIENCE: Two to three years hospital experience required; Home Care and/or Utilization Review of Case Management experience desired.
