Job summary

WelbeHealth is seeking a Case Manager, RN to guide participant care as a key voice on the interdisciplinary team (IDT). You will conduct required PACE assessments, develop personalized care plans, and support transitions of care coordination.

Work a consistent Monday through Friday schedule with no weekends and no on-call. Build relationships with participants, participate in IDT meetings, and collaborate with the WelbeHealth PACE Physician to coordinate care.

Qualifications

• Graduate of an accredited school of nursing; BSN preferred.
• Active California RN license.
• BLS and First Aid certification required.
• Two years of nursing experience with frail or elderly population preferred.
• Will obtain CPR/First Aid certification within 180 days if not currently certified.

Responsibilities

• Perform nursing assessments and participate in IDT care planning.
• Deliver and document nursing interventions and medication administration as per care plans.
• Coordinate with IDT and WelbeHealth PACE Physician to align care needs.
• Educate participants, families, and caregivers on disease processes and self-care techniques.

The Case Manager, RN directly guides participant care as a crucial voice on the interdisciplinary team (IDT). Reporting to clinical leadership, the Case Manager, RN delivers and oversees all aspects of care, including conducting required PACE assessments, developing and following personalized participant care plans, and supporting transitions of care coordination.

On the day-to-day, you will:

• Conduct clinic nursing assessments to determine participant needs, preferences, and goals, and actively participate in IDT meetings to develop participant care plans
• Deliver and document nursing interventions as agreed upon in participants’ care plans, including but not limited to maintaining a safe environment, promptly responding to physician orders, and correctly administering medications
• Collaborate with the WelbeHealth PACE Physician and other members of the IDT in coordinating participant care needs, as well as communicate with participants and their families to revise care plans
• Instruct participants, their families, and caregivers regarding the disease process, self-care techniques, and prevention strategies

Job requirements include:

• Graduate of an accredited school of nursing; Bachelor of Science (BSN) preferred
• Unencumbered California RN License
• Current Basic Life Support (BLS)/First Aide Certification required
• Two (2) years of nursing experience in a clinical setting with a frail or elderly population preferred
• Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred

Compensation consists of base salary plus bonus. WelbeHealth offers a competitive total rewards package that includes a 401(k) match, comprehensive healthcare coverage, and a broad range of additional benefits. Actual compensation will be determined based on experience and relevant qualifications.
