Job summary

Qualifications

• BSN preferred with 5 years experience in hospital/community setting.
• Must complete BSN or degree in related field within 5 years.
• RN with MSN or degree in related field with 3 years experience.

Responsibilities

• Coordinate and collaborate with physicians and multidisciplinary team on patient goals and care plan.
• Develop and maintain standards in compliance with policies and regulations.
• Maintain up-to-date knowledge to recommend new services and equipment.
• Assess and monitor plans of care with patient and family involvement.
• Adjust care plan daily based on patient condition and needs.
• Ensure accurate documentation and appropriate level of care.
• Communicate with patients, families, staff, and payors as needed.
• Assist with discharge planning and education to patients and families.
• Support in UR, QA, discharge planning, and infection control as required.
