**The Case Management Assistant responsibilities include:**

+ Assists with completing tasks necessary to facilitate patient and family access to agencies, transportation, follow up appointments, and services

+ Responsible for several activities including facilitating inbound calls and assisting patients' needs to support successful care coordination in the ambulatory setting.

+ Responsible for customer service as the initial contact for the department and quickly determines the needs of the patient to escalate calls professionally and patiently by routing patients to the appropriate discipline.

+ Performs other duties as assigned.

**Position details:**

+ **Location:** Hybrid/Tampa

+ **Status:** PRN, As Needed

+ **Schedule:** Days Vary, 8:00 AM - 4:30 PM

+ **Weekend Requirement:** Occasional

+ **On Call:** No

**Education:**

+ Required High School or Equivalent

+ Preferred Associate's

**Experience:**

+ Required 1 year in Medical Related Field
