Job summary

Saint Agnes Medical Center in Fresno, CA is seeking a Care Coordination Support role to assist the Care Coordinator with utilization review management and discharge planning tasks in an acute care setting.

You will screen for medical necessity, document criteria for admissions and continued stay, identify discharge needs, and coordinate with the Care Coordinator to implement non-complex discharges.

Qualifications

• High school diploma or equivalent is required.
• Two years of experience in an acute care hospital or clinical care setting is required.
• Well-developed interpersonal skills with the ability to communicate effectively/professionally with staff, patients, family, visitors, and physicians under routine and stressful situations are required.
• Successful completion of a medical terminology course or demonstrated functional competency is required.
• Computer data entry experience is required.
• Current American Heart Association (AHA) Healthcare Provider CPR card is required.
• Experience with case management, utilization review, and/or discharge planning processes is preferred.

Summary:

This position is responsible for assisting the Care Coordinator with utilization review management and discharge planning tasks. This includes accountability for screening and documentation for medical necessity criteria for acute care admission and continued stay certification, identifying discharge planning needs, and coordinating with the Care Coordinator for implementation of non-complex case
discharges.

Requirements:

Pay Range: $23.00 - $29.61
