St. Vincent’s Medical Center in Bridgeport, with over 3,200 employees, includes a 473-bed community teaching hospital, a 76-bed inpatient psychiatric facility, a large multispecialty provider group, and special needs services for adults and children. It is the first hospital in Fairfield County integrated into Hartford HealthCare and is the system’s second-largest hospital.
The licensed Case Coordinator - RN demonstrates expertise in a healthcare setting, assessing and treating patients along the continuum of care. This role develops and implements discharge planning within an interdisciplinary team, addressing complex clinical needs by removing barriers to post-acute care access. It collaborates with providers to identify discharge barriers, optimize hospital stay efficiency, and provides clinical leadership to ensure daily goals are met according to the patient’s plan of care.
Education: Completion of a formal Nursing program. An Associate’s degree is required; a BSN must be obtained within 6 years of hire.
Work Experience: At least three years of diversified acute-care clinical nursing experience, including medical/surgical nursing. Experience in case management, home care, or managed care is strongly considered, with preferred experience across multiple healthcare settings.
Knowledge, Skills, and Abilities: Understanding of medical terminology, disease processes, and the ability to provide comprehensive clinical documentation. Excellent communication, negotiation, and conflict resolution skills are required. Knowledge of computer applications is preferred. Ability to work collaboratively across disciplines, multi-task, and address psychosocial needs is essential. Bilingual English/Spanish is preferred.
