Wellpath is seeking a CalAIM Case Coordinator H to support successful reintegration of eligible individuals by coordinating reentry care plans under the RN Discharge Planner. The CalAIM Case Coordinator H supports the successful reintegration of CalAIM-eligible individuals by helping coordinate the development and execution of reentry care plans under the direction of the RN Discharge Planner. This role assists with Medi-Cal eligibility screens, identifies individuals who qualify for CalAIM Justice-Involved services, and helps coordinate communication and service delivery with managed care plans, providers, and community partners. The position supports continuity of care by facilitating reentry planning activities and ensuring key services and information are coordinated prior to release.
pay range: $59 - $65 / hour
