Responsibilities

Job Summary

Provides patient related care tasks based on the principles of growth and development across the life span of patients served in his/her independent unit. Assists, observes, and reports patient changes to licensed caregiver and maintains accurate medical record documentation relative to the care delivered.

  • Provides and collaborates direct patient care services under the direct supervision of a licensed caregiver. Responsibilities may include, but are not limited to personal care, activities of daily living, vital signs, intake and output, ambulation, interventions, assists/leads in delivering a wide variety of social and recreational activities and implementation of selected portions of the patient care plan as directed under supervising nurse.
  • Assists, observes and reports changes in patient condition to licensed caregiver and maintains accurate medical record documentation in relation to the care delivered.
  • Addresses and/or directs questions or concerns from patients, families, caregivers, or health care team members.
  • Follows proper CACFP and organizational procedures; organizes data; completion of reimbursement claim forms.
  • Performs other duties as assigned.

Qualifications

Education

High School Diploma or GED Required


Experience

Completion of Block 1 in Nursing OR Nursing Assistant (NA) OR six (6) months experience in a patient or resident care role OR current HonorHealth employee in good standing with 6 months of experience Required


Licenses and Certifications

Basic Life Support (BLS) Current BCLS (Basic Cardiac Life Support) card required or must be provided to ADHC and HR within 45 days from date of hire for new hires Required.

Current First Aid card required or must be provided to ADHC and HR within 30 days from date of hire for new hires. Required.

Food Service/Food Handlers Card - Current food handlers card for current employees or must obtain within 30 days of hire for new hires Required.

Fingerprint Clearance Card - Current Arizona Dept. of Health Services Level One Fingerprint Clearance Card, or shall apply and be eligible for a fingerprint clearance card within seven (7) working days of employment Required.
