Job summary

Ivy Park at Pleasanton, operated by Oakmont Management Group, seeks a compassionate Care Provider to assist residents with daily living on a 2pm–10pm shift. You will help with ADLs, maintain dignity, and support a safe, respectful environment.

No prior experience required; training provided. Must be at least 18 and able to pass background checks and health screenings. This role offers benefits and opportunities across Oakmont communities in CA, NV, and HI.

Qualifications

• Experience preferred but not required; training provided.
• Must be at least 18 years old.
• High School Diploma or GED.
• Strong desire to work with the elderly and care for their needs.
• Good written and verbal communication with staff, physicians, residents, and families.
• Must pass a Criminal Background check and Health Screening tests (TB tests).
• Vaccination and PPE may be required per regulatory guidelines.

Responsibilities

• Assist residents in Activities of Daily Living (ADL) as per care plan (eating, bathing, dressing, toileting, transferring, continence).
• Respond promptly to residents’ needs while preserving dignity, safety, and confidentiality.
• Report any change of condition to a supervisor immediately.
• Maintain a safe and secure environment following safety standards.
• Encourage teamwork and cooperative interactions with staff and other departments.

The Care Provider is responsible for providing personal assistance and daily care and services related to the residents in order to enhance the residents’ independence and quality of life, in accordance with the company’s mission and values.

Responsibilities:

• Assist residents in their Activities of Daily Living (ADL) as specified in the resident’s service and care plan. This includes eating, bathing, dressing, toileting, transferring (walking), and continence.
• Respond to residents' needs promptly while maintaining residents' self-respect, dignity, safety, and confidentiality.
• Communicate any observed or suspected resident change of condition to a supervisor immediately.
• Maintain a safe and secure environment for all staff, residents, and guests, following established safety standards.
• Encourage teamwork through cooperative interactions with co-workers and other departments

Qualifications:

• Must be at least 18 years of age
• High School Diploma or GED
• Strong desire to work with the elderly and care for their needs
• Possess written and verbal skills to interact and communicate effectively with employees, supervisors, physicians, healthcare professionals, residents, and their families.
• Must pass a Criminal Background check and Health Screening tests, including physical and TB Tests
• For the health and safety of our team members and residents, Oakmont Management Group may require team members to vaccinate, participate in daily screening, surveillance testing, and to wear face coverings and other personal protective equipment (PPE) to prevent the spread of the COVID-19 or other communicable diseases, per regulatory guidelines.
