What You Will Do:

You will help implement personalized care plans that address the whole person—mind, body, and spirit. Treat your clients like family, going above and beyond to enhance their quality of life.

Essential Functions:

• Deliver exceptional care to clients in their homes
• Assist with personalized weekly schedules and care plans
• Perform daily tasks safely and professionally (ADLs)
• Provide companionship and ongoing social & emotional support
• Assist with personal care activities, including hygiene, mobility, incontinence care, light housekeeping, feeding, and meal prep
• Maintain safety, order, and client enjoyment at all times
• Transport clients to and from appointments as needed
• Document task completion and report changes in client condition or behavior
