Job summary

Apex Companion Care, part of the LHC Group within the Optum family, is hiring PRN Home-based Caregivers to support clients in Hilliard, FL. You will assist with daily living activities, monitor health, and accompany clients to appointments while maintaining high confidentiality and safety standards.

The role requires a valid driver’s license, reliable transportation, CPR (preferred), and flexibility to work multiple shifts. Training meets Florida AHCA requirements and local regulations.

Qualifications

• Current driver’s license and vehicle insurance or access to dependable vehicle.
• Ability to function in any home situation regardless of client demographics.
• State-specific requirement: Florida training or AHCA competency test.

Responsibilities

• Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders.
• Support household tasks, meal preparation, and accompany clients to appointments or errands as needed.
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system.
• Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs.

As a home-based Caregiver, you will provide support, assistance with personal hygiene and household functions for an individual to be able to remain in their own home. We are hiring PRN Caregivers for multiple shifts in Hilliard and the surrounding areas.

Primary Responsibilities

• Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders
• Support household tasks, meal preparation, and accompany clients to appointments or errands as needed
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system
• Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs

Required Qualifications

• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client

State-Specific Requirement

• FL: Successful completion of at least forty hours of training in areas required by the Agency for Health Care Administration OR successfully passed the home health aide competency test (Form AHCA 3110-1007 created by AHCA).

Preferred Qualifications

• Current CPR certification or ability to complete within 90 days of hire
• 6+ months of experience in home care
• Ability to work flexible hours as required to meet identified clients' needs
