Overview

We are looking for caregivers, certified home health aides, and CNAs to join our team. You will be directly working with patients, following a one-on-one care plan in patients' homes.

You operate with a big heart, not only providing personalized and attentive care, but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs.

Strong verbal and written communication skills are needed to succeed in this role.

Responsibilities

• Maintain records of patient care, conditions, progress or problems to report and discuss observations with supervisor
• Assist client with full activities of daily living (ADLs)
• Assist with cooking, medication reminders, extensive transfer support (bed, chair, mobility transitions, if needed)
• Assist with ambulation (wheelchair support)
• Provide skilled services (wound care) if needed
• Hoyer Lift experience required
• Must have experience working with people with Disabilities/Amputees
• Must have a car
