Responsibilities

• Provide medication reminders per plan and duties.
• Assist with transfers, walking, and mobility as trained.
• Provide transportation and accompany clients to appointments when required.
• Follow care plans and document services accurately.

Job Summary

Responsibilities

• Assist clients with activities of daily living, including bathing, dressing, grooming, toileting, and mobility.

• Report changes in a client's condition, safety concerns, or incidents to the office promptly.
• Respect clients' privacy, personal belongings, dietary needs, and dignity at all times.
• Support clients receiving hospice or end-of-life care within the caregiver's training and assigned responsibilities.
