POSITION DESCRIPTION – LEAD STAFF

REPORTS TO: PROGRAM MANAGER / DIRECTOR

PRIMARY RESPONSIBILITY: The lead staff is a leader, mentor, and coach to the other HHA'S working in the home. The Lead DSP/Staff/caregiver will directly supervise the service delivery to individuals with physical and mental health disabilities. The Lead DSP/Staff/caregiver will ensure all services are delivered according to all agency policies. This position also provides direct care to individuals as needed. Frequent detailed communication with staff, clients, and the client’s support team is essential.

JOB RESPONSIBILITIES:
  1. Supervise service delivery, methods, and outcomes by Direct Support Professionals according to agency policies and procedures.
  2. Schedule and accompany clients to medical appointments.
  3. Train new hires and perform administrative duties such as maintaining monthly paperwork.
  4. Communicate medical and health concerns on behalf of clients.
  5. Report staffing and performance issues to the Program Manager.
  6. Conduct daily documentation checks: ISPs, Behavior reports, MARs, Staff Notes, Communication Log, Structured Leisure activities.
  7. Report daily to the Program Manager, highlighting missing documentation, client issues, or staff concerns.
  8. Assist staff in cleaning the home, including vacuuming, mopping, bathroom and kitchen cleaning, ensuring a home-like environment.
  9. Complete monthly safety checklists, report repairs, and maintain sidewalks and steps.
  10. Post weekly menus, shop for groceries accordingly.
  11. Organize structured group activities daily.
  12. Maintain client confidentiality and promptly report any safety, rights, or maltreatment concerns.
  13. Arrange transportation and attend medical, dental, psychiatric, and psychological appointments.
  14. Inventory and report hygiene supplies and medications; coordinate refills with the pharmacy.
  15. Ensure treatment orders are documented in client records.
  16. Organize client clothing in appropriate storage.
  17. Supervise safety and well-being of individuals at home.
  18. Assist with scheduling and documenting quarterly fire drills.
  19. Meet weekly with the Manager for facility audits.
  20. Maintain a safe, harassment, and discrimination-free work environment.
  21. Assist with various tasks as needed.
  22. Participate in on-call rotations at least once per month.
  23. Perform other duties as assigned.
POSITION REQUIREMENTS:
  • Use of Universal Precautions and infection control measures.
  • Knowledge of emergency procedures.
  • Report incidents/accidents promptly.
  • Participate in care conferences.
  • Maintain confidentiality.
  • Document accurately in clinical records.
  • Maintain professional boundaries and cultural awareness.
  • Plan activities to enhance socialization.
  • Assist with communication skills as needed.
  • Promote a safe, respectful environment.

  • Perform additional duties as assigned.
  • Adhere to all agency policies.
QUALIFICATIONS:
  • High school diploma or equivalent; associate degree or higher preferred.
  • Completion of a Home Health Aide course.
  • Minimum three years of experience with individuals with disabilities.
  • Must be 18+ years old.
  • Good health and emotional stability.
  • Ability to lift/move up to 50 pounds.
  • Valid driver’s license with a clean record.
  • Proficiency with computers, calculators, household appliances.
  • Strong communication skills.
WORKING CONDITIONS:

Flexible hours based on coverage needs. May be cross-trained and required to cover shifts at other sites. Regular community travel, working in various settings. Frequent interruptions, crisis management, deadlines. Exposure to potentially hostile or upset individuals, infectious diseases, odors, and dust.

Shifts available every day of the week. Compensation: $17.50 - $18.00/hour.
