Job summary

Ivy Park at Los Gatos is seeking a Caregiver / CNA / HHA to provide personal assistance and daily care to residents, focusing on independence and quality of life. Training is provided and prior experience is not required.

Responsibilities include assisting with ADLs, ensuring dignity and safety, and coordinating with the care team to meet each resident's plan. Must be 18+, HS diploma or GED, and pass background checks.

Qualifications

• Experience preferred but not necessary.
• Must be at least 18 years of age.
• High School Diploma or GED.
• Strong desire to work with the elderly and care for their needs.
• Possess written and verbal skills to interact and communicate effectively with employees, supervisors, physicians, healthcare professionals, residents, and their families.
• Criminal Background check and Health Screening tests required.

Responsibilities

• Assist residents in Activities of Daily Living (ADL) as specified in the resident's plan (eating, bathing, dressing, toileting, transferring, continence).
• Respond promptly to residents' needs while upholding dignity, safety, and confidentiality.
• Report changes in residents' condition to a supervisor.
• Maintain a safe environment following safety standards.
• Encourage teamwork with coworkers and other departments.

The Care Provider is responsible for providing personal assistance and daily care and services related to the residents in order to enhance the residents independence and quality of life, in accordance with the company‘s mission and values.

Responsibilities

• Assist residents in their Activities of Daily Living (ADL) as specified in the resident's service and care plan. This includes eating, bathing, dressing, toileting, transferring (walking), and continence.
• Respond to residents' needs promptly while maintaining residents' self-respect, dignity, safety, and confidentiality.
• Communicate any observed or suspected resident change of condition to a supervisor immediately.
• Maintain a safe and secure environment for all staff, residents, and guests, following established safety standards.
• Encourage teamwork through cooperative interactions with co-workers and other departments

• Possess written and verbal skills to interact and communicate effectively with employees, supervisors, physicians, healthcare professionals, residents, and their families.
• Must pass a Criminal Background check and Health Screening tests, including physical and TB Tests
• For the health and safety of our team members and residents, Oakmont Management Group may require team members to vaccinate, participate in daily screening, surveillance testing, and to wear face coverings and other personal protective equipment (PPE) to prevent the spread of the COVID-19 or other communicable diseases, per regulatory guidelines.
